📋 Description

• Visit Medicaid members in their homes, Assisted Living Facilities, Long-Term Care Facilities, and other care settings • Assess members’ care needs and ensure services are provided in the least restrictive setting • Identify and resolve barriers to effective care • Plan and implement interventions to meet care needs • Coordinate services and monitor and evaluate case management plans against members’ personal goals • Continuously monitor patient care and progress toward desired outcomes using assessments, data, conversations, and active care planning • Conduct telephonic and face-to-face assessments and evaluations • Remediate immediate and acute gaps in care and access • Assist enrollees with filing grievances and appeals • Provide information about MCO requirements, services, and benefits • Provide information and referrals to community resources • Guide members and families toward appropriate care resources • Facilitate interactions with payer sources, providers, interdisciplinary teams, and others involved in members’ care

🎯 Requirements

• Bachelor's degree in social sciences, social work, human services, or a related field; or unrestricted Illinois LPN with one year of experience conducting comprehensive assessments and providing formal services for the elderly; or LPC; or LSW • Must reside in Illinois in one of the listed eligible zip codes • 1+ years of experience in health care and/or case management • 1+ years of experience with Medicare and Medicaid recipients, long-term care, home and community-based services, and/or managed care organizations • Intermediate to advanced computer skills • Experience with Microsoft Word, Excel, and Outlook • Experience using electronic information applications/software, including electronic medical records • Valid state driver's license • Maintain personal vehicle liability insurance meeting state minimum limits or Humana's specified limits, whichever is higher • Pass required tuberculosis screening if selected • Dedicated home workspace without ongoing interruptions to protect member PHI/HIPAA information • Home internet with at least 25 Mbps download and 10 Mbps upload speeds • Ability to work Monday–Friday, 8am–5pm CST, with flexibility and possible additional hours based on business needs
