• Support care management activities and assigned member teams to ensure provider services, continuity of care, and member satisfaction • Conduct member outreach by telephone and home visits • Discuss care plans and service plans, next steps, resources, questions, concerns, and ongoing education with members • Coordinate care activities with healthcare and community providers, partners, members, and caregivers • Support member and provider inquiries, requests, and concerns related to care plans and service plans • Communicate with care managers, practitioners, and others to facilitate member services and continuity of care • Support service assessments and screenings and document member care needs • Maintain member records in accordance with state and regulatory requirements and distribute them to providers as needed • Follow contractual, regulatory, and organizational standards and policies • Identify needs and make referrals to Care Managers, community-based organizations, and Disease Managers • Provide education on available benefits and resources • Perform other assigned duties and comply with all policies and standards
• High School diploma or GED required • 1–2 years of related experience required • Must reside in the state of Kansas • Ability to conduct telephonic outreach and home visits • Ability to coordinate care with healthcare and community providers, partners, members, and caregivers • Ability to identify needs and make referrals • Ability to document and maintain member records according to state and regulatory requirements • Ability to follow contractual requirements, regulatory guidelines, standards of practice, and organizational policies
