📋 Description

• Support care management activities and assigned member teams • Perform member outreach by telephone or home visits • Document care and service plans and related activities • Coordinate care activities with healthcare providers, community providers, partners, members, and caregivers • Respond to member and provider inquiries, requests, and concerns • Communicate with care managers, practitioners, and others to facilitate member services and continuity of care • Support service assessments and screenings and document members’ care needs • Maintain and distribute member records to providers as needed • Provide education about available benefits and resources • Perform other duties as assigned and comply with all policies and standards

🎯 Requirements

• High School diploma or GED required • 1–2 years of related experience required • Ability to work remotely from home anywhere within the state of Missouri • Availability Monday–Friday, 8am–5pm • Ability to perform telephonic member outreach and home visits • Ability to document and maintain member records in accordance with state and regulatory requirements • Ability to follow contractual requirements, regulatory guidelines, and standards • Ability to identify needs and make referrals to Care Managers, community-based organizations, and Disease Managers
