📋 Description

• Support care management activities and assigned member teams • Perform member outreach by telephone and home visits • Document care/service plans, activities, care needs, and member records • Engage members to discuss care plans, next steps, resources, questions, concerns, and ongoing education • Coordinate care activities with healthcare and community providers, partners, members, and caregivers • Support member and provider inquiries, requests, and concerns • Communicate with care managers, practitioners, and others to facilitate services and continuity of care • Support service assessments and screenings • Follow contractual, regulatory, and state requirements and standards of practice • Identify needs and make referrals to Care Managers, community-based organizations, and Disease Managers • Provide education on available benefits and resources • Perform other assigned duties • Travel approximately 30%

🎯 Requirements

• High School diploma or GED required • 1–2 years of related experience required • For Arkansas Total Care: Bachelor's degree in social science/health-related field or high school diploma with at least one year of experience coordinating care for developmentally or intellectually disabled clients or behavioral health clients • Must reside in Arkansas or a border city • Must have a driver's license • Child and adult maltreatment check required before hire and recurring • Drug screen required at hire and recurring • Strong experience working with the Developmental Disabilities (DD) population preferred • Must reside in or be able to support members throughout Washington, Benton, and Madison counties in Arkansas • Ability to travel approximately 30%
