📋 Description

• Support care management activities and assigned member teams to ensure provider and partner services are delivered and continuity of care/member satisfaction is achieved • Perform member outreach telephonically or through home visits • Document care/service plans and related activities • Engage members to discuss care plans, next steps, resources, questions, concerns, recommended care, and ongoing education • Coordinate care activities with healthcare and community providers, partners, members, and caregivers • Support member and provider inquiries, requests, and concerns related to care/service plans • Communicate with care managers, practitioners, and others to facilitate member services and continuity of care • Support service assessments/screenings and document member care needs • Maintain member records in accordance with state and regulatory requirements and distribute them to providers as needed • Follow contractual, regulatory, and organizational standards and policies • Identify needs and make referrals to Care Managers, community-based organizations, and Disease Managers • Provide education on available benefits and resources • Perform other duties as assigned

🎯 Requirements

• High School diploma or GED required • 1–2 years of related experience required • For Arkansas Total Care: bachelor's degree in social science/health-related field, or high school diploma with at least 1 year of experience coordinating care for developmentally or intellectually disabled clients or behavioral health clients • Must reside in Arkansas or a border city • Must have a driver's license • Child and adult maltreatment check required before hire and on a recurring basis • Drug screen required at hire and on a recurring basis • Ability to support members throughout Craighead and Poinsett counties in Arkansas • Strong experience working with the developmental-disability population preferred • Field-based work supporting members with behavioral health and developmental-disability needs • Ability to travel 30%; additional travel may be required throughout Greene and Mississippi counties • Compliance with applicable policies, contractual requirements, and regulatory guidelines
