📋 Description

• Support care management activities and assigned member care teams • Perform member outreach by telephone and home visits • Discuss care plans and service plans, next steps, resources, questions, and concerns • Provide ongoing member education throughout care/service • Coordinate care with healthcare providers, community providers, partners, members, and caregivers • Support member and provider inquiries, requests, and concerns • Communicate with care managers, practitioners, and others to facilitate member services and continuity of care • Support service assessments and screenings and document member care needs • Document and maintain member records according to state and regulatory requirements • Identify needs and make referrals to Care Managers, community-based organizations, and Disease Managers • Provide education on available benefits and resources • Perform other duties as assigned

🎯 Requirements

• High School diploma or GED • 1–2 years of related experience • For Arkansas Total Care: Bachelor's degree in social science/health-related field, or high school diploma with at least one year of experience coordinating care for developmentally or intellectually disabled clients or behavioral health clients • Strong experience working with the developmental disabilities population • Must reside in Arkansas or a border city • Must be able to support members throughout Hempstead and Howard counties in Arkansas • Driver's license required • Child and adult maltreatment check required before hire and recurring • Drug screen required at time of hire and recurring • Ability to travel 30% and potentially travel throughout Little River and Sevier counties • Compliance with standards of practice, policies, contractual requirements, and regulatory guidelines
