📋 Description

• Develop, assess, and facilitate complex care management activities primarily for mental and behavioral health needs members • Evaluate member needs via phone or in-home visits and recommend or facilitate care/service plans • Perform telephonic, digital, home, and/or other site-visit outreach • Develop ongoing care plans for high-acuity members and identify needed providers, specialists, and community resources • Coordinate among members, families/caregivers, community resources, and care provider teams • Monitor care plans, member status, treatment side effects, complications, and clinical symptoms • Facilitate care coordination and collaborate with providers and specialists to ensure timely access to care • Collect, document, and maintain member information and care management activities for regulatory compliance • Educate members and families on procedures, provider instructions, treatment options, referrals, and healthcare benefits • Provide leadership feedback on opportunities to improve care quality and cost-effective delivery • Perform other duties as assigned and comply with policies and standards

🎯 Requirements

• Master’s degree in Behavioral Health or Social Work, or a degree from an accredited school of nursing • 2–4 years of related experience • Licensed Master’s Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN based on state contract requirements • BH experience required • Ability to work remotely from home anywhere within the state of Missouri • Availability Monday–Friday, 8am–5pm • Previous foster care experience preferred
