📋 Description

• Develops, assesses, and facilitates complex care management activities • Provides high quality, cost-effective healthcare outcomes • Evaluates the needs of the member and recommends the best care plan • Identifies barriers to care and provides appropriate interventions • Coordinates care between the member, family, and care provider team • Provides ongoing follow up and monitoring of member status • Collaborates with providers to revise or update care plans as necessary • Provides resource support for local services • Performs education for members on healthcare options and benefits • Collects and maintains member information for compliance

🎯 Requirements

• Requires a Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing • 2 – 4 years of related experience • RN - Registered Nurse - State Licensure and/or Compact State Licensure required
