📋 Description

• Develop, assess, and facilitate complex care management activities for primarily mental and behavioral health needs • Evaluate member needs by phone or in-home visits and recommend or facilitate care and service plans • Conduct telephonic, digital, home, and other site-visit outreach • Develop ongoing care plans for high-acuity members • Identify providers, specialists, and community resources for mental health and substance use disorder care • Coordinate among members, families, caregivers, community resources, and care provider teams • Monitor care plans, member status, treatment side effects, complications, and clinical symptoms • Facilitate care coordination and timely access to needed care and services • Collect, document, and maintain member information and care management activities for regulatory compliance • Educate members and families about procedures, provider instructions, treatment options, referrals, and healthcare benefits • Provide leadership feedback on opportunities to improve care quality and cost-effectiveness • Comply with all policies and standards • Perform other duties as assigned

🎯 Requirements

• Applicants must reside in Georgia • Active RN license or Licensed Master’s Behavioral Health Professional credential required, such as LCSW, LMSW, LMFT, LMHC, or LPC • Master’s degree in Behavioral Health or Social Work, or a degree from an accredited school of nursing • 2–4 years of related experience • Behavioral health experience required for the applicable RN or Licensed Master’s Behavioral Health Professional credential • Strong time management and assessment skills • Telephonic experience • Excellent communication and customer service skills • Behavioral health knowledge • Strong organizational skills • Comfortable working with computers
