• Develop, assess, and coordinate care management activities based on member needs • Develop or contribute to personalized care plans/service plans • Educate members and families/caregivers on services and benefit options • Evaluate member needs, barriers to care, and available resources • Recommend and facilitate plans for the best outcome • Identify providers, specialists, and community resources needed for care • Provide psychosocial and resource support for employment, education, housing, food, participant direction, independent living, justice, and foster care • Coordinate between members/families/caregivers and care provider teams • Monitor progress toward care plan goals and member status or changes in condition • Collaborate with healthcare providers on care plan revisions and identified member needs • Collect, document, and maintain member information and care management activities for regulatory compliance • Perform on-site visits when appropriate • Provide education on procedures, healthcare provider instructions, care options, referrals, and healthcare benefits • Perform other duties as assigned and comply with policies and standards
• Bachelor’s degree • 2–4 years of related experience • Equivalent experience acquired through applicable knowledge, duties, scope and skill reflective of the level of this position may be accepted • Graduate from an Accredited School of Nursing if holding clinical licensure • Current state clinical license preferred • Previous case management or foster care experience strongly preferred • Missouri LPN license preferred • Ability to work Monday–Friday, 8am–5pm • Applicants must be able to work remotely from their home anywhere within the state of Missouri
