📋 Description

• Develop, assess, and coordinate care management activities based on member needs • Develop or contribute to personalized care plans/service plans • Educate members and families/caregivers on services and benefit options • Evaluate member needs, barriers to care, available resources, and recommend or facilitate plans • Identify providers, specialists, and community resources needed for care • Provide psychosocial and resource support for employment, education, housing, food, participant direction, independent living, justice, and foster care needs • Coordinate members/families/caregivers with care provider teams to ensure timely access to care and services • Monitor progress toward care plan goals and member status or changes in condition • Collaborate with healthcare providers on care plan revisions and identified member needs • Collect, document, and maintain member information and care management activities for regulatory compliance • Perform on-site visits to assess member needs and collaborate with providers or resources • Provide education on procedures, healthcare provider instructions, care options, referrals, and healthcare benefits • Perform other duties as assigned and comply with all policies and standards

🎯 Requirements

• Bachelor’s degree and 2–4 years of related experience • Alternatively, equivalent experience acquired through accomplishments of applicable knowledge, duties, scope and skill reflective of the level of this position • If holding clinical licensure, graduation from an Accredited School of Nursing • Current state clinical license preferred • Excellent communication and customer service skills • Experience working with multidisciplinary teams • Strong problem-solving abilities • Computer proficiency • Effective organizational and time-management skills • Ability to work independently as a self-starter • Must reside in Montgomery County, Ohio, or Butler, Warren, Greene, Clinton, Clark, Miami, or Darke County • Ability to work Monday through Friday, 8:00 AM to 5:00 PM
