📋 Description

• Lead community health workers, social workers, and resource coordinators across Georgia • Manage care coordination activities for assigned members • Ensure appropriate services are delivered and continuity of care is maintained across services • Manage care coordination team members and escalated member/provider issues • Monitor and review member support care coordination activities, including transitions of care • Oversee and approve team caseloads based on state and contractual requirements and member needs • Partner with care coordination management team leaders to address member needs • Ensure standards of practice and policies comply with contractual and regulatory requirements • Set goals and objectives for the care coordination team • Provide updates and insights on team goals • Give feedback to team members and stakeholders to improve services • Educate and guide the team on member/provider issues and concerns • Assist senior leadership with onboarding, hiring, and training • Manage telephonic, digital, home, and other site outreach as needed • Ensure adherence to service-level, quality, and KPI expectations • Lead and champion change within the scope of responsibility • Perform other duties as assigned

🎯 Requirements

• Candidates must reside in Georgia • Strong Medicaid experience • Bachelor's degree • 5+ years of related experience • Equivalent experience may be acquired through applicable knowledge, duties, scope and skill reflective of the level of this position • Ability to work Monday through Friday, approximately 8:00 a.m. to 5:30 p.m., with flexibility to support staff working until 6:00 p.m. • Approximately 25% statewide travel • Comply with all policies and standards
