📋 Description

• Provide resource support to members and connect them to local resources based on care/service assessments and plans • Help assess member needs and develop or contribute to care plans/service plans • Identify and coordinate referrals for care and services • Connect members to local resources, obtain required documentation and support application processes • Determine member eligibility and care needs based on assessments and care plans/service plans • Monitor care/service delivery and follow up with members, caregivers and providers • Work with providers, community organizations, stakeholders and governmental entities to help members and families access essential services • Assist with developing education and training programs for care management staff and providers • Support development of local resource service materials, policies and procedures • Ensure resource services information is documented and up to date • Conduct telephonic and in-person outreach to locate hard-to-reach individuals and families • Perform other assigned duties and comply with policies and standards

🎯 Requirements

• Must be authorized to work in the U.S. without the need for employment-based visa sponsorship now or in the future • Candidates must reside in North Carolina • Bachelor's degree and 2–4 years of related experience, or equivalent experience acquired through applicable knowledge, duties, scope and skill • Ability to support members with employment, housing, independent living, justice, foster care and/or community transition services • Ability to work with care teams, providers, community-based organizations, community stakeholders and governmental entities • Ability to conduct telephonic and in-person outreach • Compliance with all policies and standards
