📋 Description

• Evaluate pharmacotherapy suitability for high-risk patients with chronic conditions including diabetes, hypertension, heart failure, and COPD • Execute clinical protocols allowed by the organization and state regulatory agencies • Optimize drug coverage benefits, formulary products, lower-cost alternatives, and home delivery pharmacy • Collaborate with physicians, nurses, and other healthcare professionals • Participate in multidisciplinary high-risk rounds and adjust treatment plans • Counsel patients and communicate complex medical information • Provide disease-state education and support medication self-management • Participate in medication safety and efficacy quality-improvement initiatives • Perform post-discharge medication reconciliation for high-risk transitions of care • Document clinical activity in clinic EMR and pharmacy or clinical management systems • Support CMS Stars and HEDIS quality measures • Maintain documentation of clinical interventions and patient care activities • Conduct in-service education and training for healthcare staff • Develop and share pharmacotherapy educational materials • Stay current with pharmacotherapy advancements and clinical guidelines • Consult healthcare providers on complex medication management issues • Provide support within the pharmacist’s scope of practice to advance organizational goals • Implement targeted initiatives directed by Pharmacy leadership according to best practices and regulatory standards • Report to the Associate Director Integrated Clinic Pharmacist • Support the full physician panel across health plans and potentially multiple clinic locations

🎯 Requirements

• Active and unrestricted pharmacist license in the state of Florida • Willingness to become licensed in additional states for market support • Pharm.D. or other advanced pharmacy degree • Minimum five (5) years of experience as a practicing pharmacist • Experience working in a team environment and supporting common goals • Proficiency using MS Office programs such as Word, Excel, PowerPoint, Teams, and Outlook • Ability to troubleshoot and resolve basic technical needs within a remote environment • Must reside within a 50-mile radius of Englwood Park, West Oakland, West Orange, or Plant City • Ability to work Monday through Friday, 8 a.m.–5 p.m. Eastern • Minimum home internet speed of 25 Mbps download and 10 Mbps upload • Dedicated workspace without ongoing interruptions to protect member PHI/HIPAA information • Clinical residency in ambulatory care or similar discipline preferred • Board certification in geriatric pharmacy preferred • Experience in an ambulatory care/primary practice setting preferred • Familiarity with electronic medical record (EMR) systems preferred • Experience managing drug therapy for patients with chronic conditions preferred • Ability to identify and implement innovative solutions for improved efficiencies and outcomes preferred • Multilingual, preferably English/Spanish, preferred
