📋 Description

• Support members’ optimal wellness by coordinating services and resources • Identify and resolve barriers that hinder effective care • Monitor patient care using assessments, data and conversations with members • Ensure patients progress toward desired outcomes • Apply professional concepts, standards, regulations, strategies and operating standards • Make decisions regarding work approach and priorities while following direction • Report directly to the Manager, Care Management

🎯 Requirements

• 2 or more years of related experience • Proficiency with Outlook, Word, Excel, SharePoint and Teams • Ability to use a variety of electronic information applications/software programs • Exceptional verbal/written communication and interpersonal skills • Passion for contributing to an organization focused on continuously improving consumer experiences • Minimum home internet speed of 25 Mbps download and 10 Mbps upload • Dedicated workspace without ongoing interruptions to protect member PHI / HIPAA information
