📋 Description

• Advocate and educate new patients from the initial intake phone assessment through preparation for the first visit with the onsite care team • Navigate new patients through the new patient intake process using assessment, planning, and evaluation • Evaluate whether patients are clinically appropriate for safe travel to City of Hope • Liaise between patients, families, multidisciplinary teams, and providers to coordinate first appointments and streamline initial evaluations • Assess cancer treatment history, including diagnosis, treatments, physicians, and facilities • Work with OIS representatives and new patient schedulers to evaluate appropriateness for visiting the organization • Interview patients to assess clinical appropriateness for care at affiliated facilities and clinical trials • Complete preadmission clinical evaluations and recommend care based on diagnosis, providers, treatment options, and financial resources • Ensure timely documentation in the EMR and receipt of current outside records and imaging • Follow patients through treatment decisions, identify and resolve barriers, and prevent gaps in care • Facilitate or assist with admissions through the intake process • Collaborate with intake, medical oncology, specialty clinics, and CAM teams to develop plans of care and educate patients and families • Facilitate multidisciplinary patient education and address patient and family concerns • Act as liaison between Oncology Information Services and Medical Center care providers for service recovery • Support department goals and performance improvement initiatives • Attend required staff meetings and plan workflow for timely completion of responsibilities • Ensure encounters and new charts are complete with necessary physician documentation • Assess special travel needs and coordinate details with support staff • Apply problem-solving techniques to the intake evaluation process • Demonstrate knowledge of patient safety goals, variance reporting, disaster codes, hand hygiene, and transmission-based precautions • Adhere to facility policies, procedures, and Joint Commission standards

🎯 Requirements

• Graduate of an accredited school of nursing • State-issued Registered Nursing (RN) license • Successful completion and verification of the California state registered nursing license within 90 days of employment • BLS • Oncology Certified Nurse highly preferred • Ability to work Monday–Friday, 8-hour shifts between 7 a.m. and 7 p.m. Pacific Standard Time • Knowledge of patient safety goals relevant to the work environment • Knowledge of hospital variance reporting systems and appropriate management procedures • Knowledge of organizational disaster codes • Completion of annual Hazardous Communications Training • Completion of required CE Direct • Compliance with organizational hand hygiene and standard/transmission-based precaution guidelines • Adherence to facility policies and Joint Commission standards
