• Provide compassionate telephonic care and clinical direction during evenings, nights, weekends, and holidays • Serve as the first line of support for residents and facility staff after-hours • Provide medical direction and care over the phone • Use Curana’s telephonic platform to take and place calls • Coordinate care between facilities, hospitals, and clinics • Deliver high-quality, cost-effective care addressing acute, chronic, and behavioral health needs • Collaborate with physicians and specialty providers • Perform at least 30% telehealth visits, including evaluation of acute changes, falls, and controlled substance visits • Manage high call volumes and multiple calls independently across several states • Perform comprehensive assessments and accurately document encounters in the EMR • Ensure compliance with CMS requirements • Apply clinical protocols and practice guidelines to support safe treatment in place whenever possible • Participate in mandatory education and training • Provide weeknight coverage from 5pm–8am local time • Provide continuous weekend coverage from Friday at 5pm to Monday at 8am • Provide holiday coverage from 5pm at the end of the last business day to 8am at resumption of business hours • Work a set schedule including weeknight 12-hour or 6-hour night shifts and every-other-weekend 12-hour day shifts • Work overnight shifts and four 12-hour holiday shifts per year
• Master's Degree as a Nurse Practitioner • Current unrestricted NP license in Iowa, Missouri, or Ohio; must be licensed in 2 of these states for consideration • Ability to obtain further multi-state licenses, including but not limited to Illinois, Kansas, Indiana, Minnesota, Nebraska, and Wisconsin • Nurse Practitioner national certification as ANP, FNP, or GNP • Ability to obtain DEA licensure / Prescriptive Authority • Background in acute and chronic disease management • Clinical background in adult, family, or geriatrics • 3+ years of experience as a NP • Ability to gain a collaborative practice agreement, if applicable in the applicable state(s) • Ability to work scheduled shifts in accordance with scheduling policies • Proficient computer skills, including the ability to document medical information with written and electronic medical records • Experience working in a nursing home, or with seniors in an acute care facility (preferred) • Understanding of geriatrics, chronic illness, and acute disease management (preferred) • Understanding of advanced illness and end-of-life discussions (preferred) • Ability to develop and maintain positive customer relationships (preferred) • Adaptability to change (preferred) • Company is unable to provide sponsorship for a visa at this time (H1B or otherwise)
