📋 Description

• Conduct health evaluations to assess substance use, co-occurring conditions, and overall functioning • Diagnose health conditions and create individualized treatment plans • Prescribe and manage medications, monitor for side effects, and adjust treatment as needed • Collaborate with the Holon care team and external providers to deliver coordinated, whole-person care • Serve as a clinical consultant on complex or high-need cases • Educate clients and families on treatment plans, medications, and recovery support strategies • Maintain accurate and timely documentation in compliance with ethical, legal, and medical standards • Participate in team meetings, professional development, and continuous training • Provide telehealth-based assessments, medication management, and care coordination for clients with SUD • Work with Care Navigators, Recovery Coaches, Case Managers, judicial partners, and community-based organizations

🎯 Requirements

• Current residence in Washington State • Active and unrestricted NP license in Washington • Active DEA license (and CDS where applicable) • Several years of experience in behavioral health, addiction medicine, or psychiatry • Board certification in specialty preferred • Strong documentation skills and proficiency in EHR platforms • Experience in addiction medicine or psychiatry • Compassion toward individuals living with SUD and mental health challenges • Ability to collaborate in integrated and team-based care models • Organized, flexible, and capable of navigating complex systems • Clear and confident virtual communication skills • Current Washington NP license and residence in Washington State are required
