📋 Description

• Conduct telephonic outreach to and enroll eligible Medicaid members • Engage members through inbound and outbound interactions to address needs and encourage behavioral change • Provide healthcare navigation and health plan customer support • Conduct direct member outreach and engagement • Facilitate case assignment and connect identified members to care managers through warm transfers • Support implementation of care plans and promote effective utilization of healthcare services • Promote and support quality effectiveness • Answer telephone calls, take messages, research information, and assist in solving problems • Follow company policies, procedures, and regulatory standards • Maintain accurate and complete documentation meeting risk management, regulatory, and accreditation requirements • Protect member confidentiality

🎯 Requirements

• Demonstrates ability to meet daily metrics with speed, accuracy and a positive attitude • Effective communication, telephonic and organization skills • Ability to manage multiple priorities and adapt to change • Strong customer service skills and attention to customer needs • Ability to identify and resolve issues and promote positive member outcomes • Knowledge of care management processes, privacy and confidentiality, quality management processes, regulatory and accreditation guidelines, and company policies and procedures • Ability to complete detailed electronic documentation for each member call • Ability to work independently and meet deliverables and deadlines • Ability to participate effectively in a multi-disciplinary team • Experience with computers, including Microsoft Word, Outlook, and Excel, preferred • Four years’ experience in a healthcare field, preferred • Familiarity with basic medical terminology and care management concepts, preferred • Flexibility to work occasional nights and weekends outside standard business hours spanning 8:00 am to 8:00 pm • Strong verbal and written communication skills • Bilingual Spanish preferred • High School diploma, G.E.D. or equivalent
