📋 Description

• Coordinate, document, and communicate all aspects of the utilization/benefit management program • Apply critical thinking and knowledge of clinically appropriate treatment, evidence-based care, and medical necessity criteria • Provide care coordination, support, and education using care management tools and resources • Conduct comprehensive evaluations of referred members’ needs and eligibility • Recommend case resolution approaches by evaluating benefit plans and available programs and services • Identify high-risk factors and service needs and make appropriate referrals • Coordinate and implement assigned care plan activities and monitor progress • Use a holistic approach to overcome barriers and meet goals • Present cases at case conferences for multidisciplinary review • Identify and escalate quality-of-care issues • Negotiate appropriate options and services to meet members’ benefits and healthcare needs • Use influencing and motivational interviewing skills to promote member engagement and healthy lifestyle changes • Provide coaching, information, and support for independent medical and healthy lifestyle choices • Help members participate knowledgeably in healthcare decision-making with providers • Follow case management processes, regulatory and accreditation guidelines, and company policies • Travel within Maricopa County, Arizona, to visit members

🎯 Requirements

• 2+ years of experience in case management • Experience working with people designated as having a serious mental illness (SMI) • Experience working with elderly people • Must reside in West Valley, Arizona • Ability to travel up to 50% of the time within West Valley, Arizona • Bachelor’s degree or licensed RN (Registered Nurse), valid in the state of Arizona • Bachelor’s degree in Social Work, Psychology, Special Education, or Counseling preferred • Previous experience working with members with a physical disability preferred • Strong organizational and time management skills preferred • Ability to collaborate with internal and external partners preferred • Demonstrated proficiency in Microsoft Office Suite, including Outlook and Word, preferred • Previous experience collaborating with medical professionals preferred • Bilingual (English/Spanish) preferred
