📋 Description

• Coordinate all case management activities with members • Evaluate members' medical needs • Facilitate the overall wellness of members • Develop strategies to address issues, outcomes, and opportunities to enhance members' overall wellness through integration • Instruct programs and procedures in compliance with network management and clinical coverage policies • Travel within 125 miles round trip for 1 day in the field

🎯 Requirements

• Must reside in Louisiana in Region 7: Caddo, Bossier, Webster, Claiborne, Bienville, Red River, DeSoto, Sabine, or Natchitoches parishes • Current/Active Clinical Licensure (RN, LCSW, LPC, LMFT) • Case Management experience • Strong customer service orientation and problem solving • Excellent Communication/Telephonic skills • Excellent motivational interviewing skills/ability to build rapport and trust telephonically • Excellence in documentation and respecting compliance/regulatory standards • Highly organized and self-driven without direct supervision • Ability to manage time effectively • Ability to achieve performance metrics • Technology proficiency • Ability to travel within 125 miles round trip for 1 day in the field • Ability to work a variable schedule, including 2 days weekly until 9pm local time • Bachelor's and/or Master's Degree listed under education requirements • CCM Certification preferred • Previous experience in Managed Care Organization preferred
