📋 Description

• Complete In-home Evaluations, Focused Visits, and Video Visits • Exemplify Signify Health operational, clinical, and member experience KPIs • Test new Diagnostic & Preventive Services and clinical workflow changes and provide feedback • Dedicate approximately 20% of time to peer coaching and mentorship, with up to 80% spent completing visits • Provide dedicated guidance to new-hire clinicians during their first 90 days • Facilitate virtual and in-person shadowing opportunities • Provide best-practice guidance on clinical workflows and diagnostic and preventive services • Offer group support to active clinicians through webinars and/or office hours • Provide real-time support to in-field clinicians • Serve as the first line of response for clinical inquiries and escalate issues to Clinical Leaders when needed • Escalate operational issues to Operations Managers when needed • Meet regularly with Clinical Leaders for support and training • Participate in manager, team, and clinical meetings • Serve as an early adopter for new diagnostic, preventive, and workflow changes • Maintain required training, affiliations, compliance, supplies, and diagnostic equipment • Travel in-region and locally as required • Accommodate field observations and ride-along experiences

🎯 Requirements

• Clinical background and state licensure (e.g., MD/DO, NP/PA) • Ability to sit, stand, bend, reach, push, pull and lift up to 20 pounds of supplies • Ability to provide 40 hours of availability weekly • Ability to consistently provide 128+ capacity units per month • Ability to consistently complete 88+ visits per month • Ability to provide mentoring and coaching support, including rotating evenings and weekends as assigned • Maintain training, affiliation, and compliance with Video Visit requirements • Complete diagnostic and preventive service (DPS) products as directed • Maintain required supplies and associated diagnostic equipment • Flexibility to travel in-region and locally as dictated by business needs • Ability to accommodate field observations and ride-along experiences • Ability to effectively inspire, motivate, and mentor peers • Ability to share leading practices, tips, and tricks from first-hand experiences • Understanding of inquiry management and escalation of performance issues to leadership • Understanding of how to uphold high-quality member care across teams • Experience completing IHEs according to operational, clinical, and member experience KPIs • Experience mentoring teams and providing 1:1 professional and personal support • Experience with Signify IHE tools and processes • Experience developing and presenting group presentations • Preferred: Multi-state licensure • Preferred: 5–7 years of clinical experience with a strong background in primary care, chronic disease management, or related field • Preferred: Minimum 6 months performing Signify Health in-home health evaluations
