📋 Description

• Complete In-home Evaluations, Focused Visits, and Video Visits • Meet Signify Health operational, clinical, and member experience KPIs • Test new Diagnostic & Preventive Services and clinical workflow changes and provide feedback • Spend up to 80% of the week completing visits and 20% mentoring peers • Reserve one assigned day per week for coaching and mentorship activities • Provide dedicated guidance to new hire clinicians during their first 90 days • Facilitate virtual and in-person shadowing opportunities • Provide best-practice guidance on clinical workflows and diagnostic and preventive services • Offer group support to active clinicians through webinars and office hours • Provide real-time support to in-field clinicians • Serve as the first line of response for non-urgent clinical inquiries and escalate issues to Clinical Leaders when needed • Escalate operational issues to Operations Managers • Meet regularly with Clinical Leaders for support and training • Participate in manager, team, and clinical meetings and special events as needed • Provide mentoring and coaching support during assigned rotating evenings and weekends • Maintain required supplies and diagnostic equipment • Accommodate field observations and ride-alongs • Perform additional duties and responsibilities as required

🎯 Requirements

• Clinical background and state licensure (e.g., MD/DO, NP/PA) • Ability to sit, stand, bend, reach, push, pull, and lift up to 20 pounds of supplies • Ability to provide 40 hours of availability weekly • Ability to consistently provide 128+ capacity units per month • Ability to consistently complete 88+ visits per month • Experience completing IHEs according to Signify Health operational, clinical, and member experience KPIs • Experience mentoring teams and providing 1:1 professional and personal support • Experience with Signify IHE tools and processes • Experience developing and presenting group presentations • Ability to manage inquiries and escalate performance issues to leadership • Ability to uphold high-quality member care across teams • Preferred: Multi-state licensure • Preferred: 5–7 years of clinical experience with a strong background in primary care, chronic disease management, or related field • Preferred: Minimum 6 months performing Signify Health in-home health evaluations • Regular training and compliance with Video Visit requirements • Regular training and completion of all directed diagnostic and preventive service products • Ability to travel in-region and locally as business needs dictate • Ability to accommodate field observations and ride-along experiences as requested
