📋 Description

• Conduct approximately 45-minute in-home health evaluations • Build trusted relationships with health plan members • Review medications and medical history • Perform physical evaluations • Perform additional diagnostic tests when ordered by the health plan, such as diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Travel locally and within licensed states as needed • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services • Help connect members to appropriate care and improve health outcomes

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Able to cover Colorado Springs / El Paso County • Able to work safely and comfortably in homes that may contain household pets, secondhand smoke, and airborne allergens or irritants • Willingness and ability to travel locally and within licensed states as needed • Ability to conduct virtual visits as needed • Ability to provide Diagnostic & Preventive Services, including spirometry and diabetic eye exams • Multi-state licenses (preferred)
