📋 Description

• Provide approximately 45-minute in-home health evaluations • Engage with health plan members in their homes and build trusted relationships • Review medications and medical histories • Perform physical evaluations • Conduct additional diagnostic tests when ordered by the health plan • Use an iPad and clinical workflow for each evaluation • Visit people nationwide to connect them with appropriate care • Travel locally and within licensed states as needed • Conduct virtual visits as needed • Provide Signify Health diagnostic and preventive services, including spirometry, diabetic eye exams, and bone density screening • Educate members and answer their health questions • Do not prescribe medicine, order lab tests, or alter current treatment regimens

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Multi-state licenses (preferred) • Nurse Practitioner or Physician Assistant qualification • Ability to travel locally and within licensed states as needed • Ability to conduct virtual visits as needed • Ability to safely perform duties in homes containing household pets, secondhand smoke, and potential airborne allergens or irritants • Ability to provide diagnostic and preventive services, including spirometry, diabetic eye exams, and bone density screening
