📋 Description

• Provide approximately 45-minute in-home health evaluations • Engage one-on-one with health plan members and answer health questions • Review medications and medical history • Perform physical evaluations • Provide additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services products • Build trusted relationships and help improve health outcomes • Make referrals to members’ health plans and encourage involvement of loved ones • Ensure primary care physicians are aware of members’ health and quality of life

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Able to cover Traverse City and the specified counties: Antrim, Benzie, Grand Traverse, Kalkaska, Lake, Leelanau, Manistee, Mason, Missaukee, Osceola, and Wexford • Able to travel locally and within licensed states, as needed • Able to conduct virtual visits as needed • Able to safely perform duties in homes that may contain household pets, secondhand smoke, and airborne allergens or irritants • MD or DO qualification • Multi-state licenses preferred
