📋 Description

• Provide in-home health evaluations in members’ residences • Conduct approximately 45-minute visits • Review medications and medical history • Perform physical evaluations • Perform additional diagnostic tests when ordered by the health plan • Answer health questions and provide health education • Identify connections to appropriate care and help bridge gaps in care • Travel locally and within licensed states as needed • Conduct virtual visits as needed • Provide Signify Health diagnostic and preventive services

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification required • Multi-state licenses preferred • Nurse Practitioner or Physician Assistant qualification • Ability to travel locally and within licensed states as needed • Ability to conduct visits in homes that may contain household pets, secondhand smoke, and airborne allergens or irritants • Ability to use an iPad and clinical workflow • Ability to provide diagnostic and preventive services, including spirometry, diabetic eye exams, and bone density screening
