📋 Description

• Provide approximately 45-minute in-home health evaluations • Engage with health plan members and build trusted relationships • Review medications and medical histories • Perform physical evaluations • Provide additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry • Conduct virtual visits as needed • Provide Signify Health services, including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening • Travel locally and within licensed states as needed • Help identify connections to appropriate care and improve health outcomes • Work as part of a national network of purpose-driven clinicians

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Multi-state licenses (preferred) • Ability to travel locally and within licensed states, as needed • Ability to conduct visits in private residences that may contain household pets, secondhand smoke, and other airborne allergens or irritants • Ability to use an iPad and clinical workflow for evaluations • Nurse Practitioner or Physician Assistant qualification
