📋 Description

• Provide In-Home Health Evaluations in members’ homes • Travel primarily up to 100% of the time across a defined coverage area • Conduct approximately 45-minute visits • Review medications and medical histories • Perform physical evaluations • Perform ordered diagnostic tests such as diabetic eye exams, spirometry, and bone density screenings • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Diagnostic & Preventive Services products • Build trusted relationships and help bridge gaps in care • Make referrals to members’ health plans and encourage loved ones’ participation • Be flexible to travel locally and within licensed states

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Able to obtain and maintain multi-state licenses in coverage area, as required • MD or DO qualification • Able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants • Able to travel locally and within licensed states • Able to conduct virtual visits as needed
