📋 Description

• Provide in-home health evaluations in patients’ residences • Travel locally and within licensed states, potentially requiring overnight hotel stays, car rentals, and/or flights • Conduct approximately 45-minute visits and build trusted relationships with members • Review medications and medical histories • Perform physical evaluations • Provide additional diagnostic and preventive services such as diabetic eye exams, spirometry, and bone density screening when ordered • Conduct virtual visits as needed • Use an iPad and intuitive clinical workflow for each evaluation • Help identify care gaps and connect people to appropriate care • Make referrals to health plans and encourage participation of patients’ loved ones • Ensure primary care physicians are aware of patients’ health and quality of life

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Able to obtain and maintain multi-state licenses in coverage area, as required • MD or DO qualification • Ability to travel locally and within licensed states • Ability to travel up to 100% of the time across a defined coverage area • Ability to safely perform duties in private residences that may contain household pets, secondhand smoke, allergens, or irritants • Ability to conduct virtual visits as needed • Ability to use an iPad and clinical workflow • Ability to complete medication and medical history reviews, physical evaluations, and diagnostic tests as indicated
