📋 Description

• Provide approximately 45-minute in-home health evaluations • Engage one-on-one with health plan members and answer health questions • Review medications and medical histories • Perform physical evaluations • Provide diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Travel locally and within licensed states • Build trusted relationships and help improve health outcomes • Identify care needs and make referrals to health plans • Encourage participation of members’ loved ones • Ensure primary care physicians are aware of members’ health and quality of life

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Multi-state licenses (preferred) • MD or DO qualification • Ability to travel locally and within licensed states, as needed • Ability to conduct virtual visits as needed • Ability to safely perform duties in homes that may contain household pets, secondhand smoke, and airborne allergens or irritants • Ability to perform in-home health evaluations, including medication and medical history review, physical evaluation, and diagnostic and preventive services
