📋 Description

• Provide in-home health evaluations for people in the comfort of their homes • Conduct approximately 45-minute visits • Engage one-on-one and answer health questions • Review medications and medical history • Perform physical evaluations • Provide additional diagnostic tests when ordered by the person’s health plan, including diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Do not prescribe medicine, order lab tests, or alter current treatment regimens • Visit people nationwide as part of efforts to connect them to appropriate care • Work with Signify Health as a clinician colleague • Travel locally and within licensed states as needed • Conduct virtual visits as needed • Provide Diagnostic & Preventive Services, including spirometry, diabetic eye exams, and bone density screening • Make referrals to individuals’ health plans and encourage participation of loved ones • Ensure primary care physicians are aware of patients’ health and quality of life

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Multi-state licenses (preferred) • MD or DO qualification • Ability to travel locally and within licensed states, as needed • Ability to conduct visits in private residences that may contain household pets, secondhand smoke, and airborne allergens or irritants • Ability to use an iPad and clinical workflow for evaluations
