📋 Description

• Provide In-Home Health Evaluations in members’ homes • Conduct approximately 45-minute visits, including medication and medical history review and physical evaluation • Perform additional diagnostic tests when ordered by the health plan, including diabetic eye exams, spirometry, and bone density screening • Use an iPad and clinical workflow for each evaluation • Answer health questions and build trusted relationships with members • Help identify gaps in care and connect people to appropriate care • Conduct virtual visits as needed • Provide Signify Health Diagnostic & Preventive Services products • Travel locally and within licensed states as needed • Visit health plan members in diverse home environments

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Multi-state licenses (preferred) • MD or DO qualification • Ability to travel locally and within licensed states, as needed • Ability to conduct virtual visits as needed • Ability to safely perform duties in homes containing household pets, secondhand smoke, and potential airborne allergens or irritants • Ability to perform in-home visits and work in members’ private residences
