📋 Description

• Provide in-home health evaluations for health plan members • Conduct approximately 45-minute visits, including one-on-one health discussions • Review medications and medical history • Perform physical evaluations • Provide additional diagnostic tests when ordered, including diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Provide Signify Health Diagnostic & Preventive Services, including spirometry, diabetic eye exams, and bone density screening • Travel locally and within licensed states as needed • Conduct virtual visits as needed • Build trusted relationships and help bridge gaps in care • Make referrals to health plans and encourage participation of members' loved ones when appropriate • Ensure primary care physicians are aware of members' health and quality of life

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • MD or DO qualification • Ability to travel locally and within licensed states, as needed • Ability to conduct virtual visits as needed • Ability to safely and comfortably perform duties in homes that may contain household pets, secondhand smoke, and other airborne allergens or irritants • Multi-state licenses preferred • Availability for 40 hours per week
