📋 Description

• Provide in-home health evaluations for health plan members • Conduct approximately 45-minute visits, including medication and medical history reviews and physical evaluations • Perform additional diagnostic tests when ordered by the person’s health plan, including diabetic eye exams, spirometry, and bone density screenings • Use an iPad and clinical workflow for each evaluation • Build trusted relationships and help improve health outcomes • Visit people nationwide and connect them to appropriate care • Travel locally and within licensed states • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services products • Do not prescribe medicine, order lab tests, or alter current treatment regimens

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • MD or DO qualification • Ability to travel locally and within licensed states, as needed • Ability to conduct virtual visits as needed • Ability to safely and comfortably perform duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants • Multi-state licenses (preferred)
