📋 Description

• Provide In-Home Health Evaluations in members’ homes • Conduct approximately 45-minute visits • Engage one-on-one with members and answer health questions • Review medications and medical history • Perform physical evaluations • Provide additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Visit people nationwide to connect them with appropriate care • Conduct virtual visits as needed • Provide Signify Health services, including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening • Make referrals to members’ health plans and encourage loved ones’ participation • Ensure primary care physicians are aware of members’ health and quality of life • Work as a clinician colleague with Signify Health

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Ability to travel locally and within licensed states, as needed • Ability to conduct virtual visits as needed • Ability to perform duties safely and comfortably in homes that may contain household pets, secondhand smoke, and airborne allergens or irritants • MD or DO qualification • Multi-state licenses preferred
