📋 Description

• Provide in-home health evaluations in members’ residences • Conduct approximately 45-minute visits including medication and medical history review and physical evaluation • Perform additional diagnostic tests when ordered by the person’s health plan, such as diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Avoid prescribing medicine, ordering lab tests, or altering current treatment regimens • Build trusted relationships and help improve health outcomes • Visit people nationwide and connect them to appropriate care • Provide Signify Health services, including diagnostic and preventive services • Travel locally and within licensed states • Conduct virtual visits as needed

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Multi-state licenses (preferred) • MD or DO qualification • Ability to travel locally and within licensed states, as needed • Ability to conduct virtual visits as needed • Ability to safely and comfortably perform duties in homes that may contain household pets, secondhand smoke, and airborne allergens or irritants
