📋 Description

• Provide in-home health evaluations lasting approximately 45 minutes • Engage one-on-one with health plan members and answer health questions • Review medications and medical history • Perform physical evaluations • Conduct additional diagnostic tests when ordered by the health plan • Help identify and bridge gaps in care • Visit people nationwide and connect them with appropriate care • Travel locally and within licensed states as needed • Conduct virtual visits as needed • Provide Signify Health Diagnostic & Preventive Services • Build trusted relationships with health plan members • Work as a clinician colleague with Signify Health

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Multi-state licenses preferred • MD or DO qualification • Ability to travel locally and within licensed states as needed • Ability to conduct virtual visits as needed • Ability to perform duties safely and comfortably in private residences that may contain household pets, secondhand smoke, airborne allergens, or irritants • Ability to provide Diagnostic & Preventive Services, including spirometry, diabetic eye exams, and bone density screening • Ability to use an iPad and clinical workflow • Ability to work 40 hours per week
