📋 Description

• Provide In-Home Health Evaluations in members’ homes • Conduct approximately 45-minute visits • Engage one-on-one with members and answer health questions • Review medications and medical history • Perform physical evaluations • Provide additional diagnostic tests when ordered by the member’s health plan, including diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Visit people nationwide to connect them with appropriate care • Travel locally and within licensed states as needed • Conduct virtual visits as needed • Provide Signify Health Diagnostic & Preventive Services, including spirometry, diabetic eye exams and bone density screening • Make referrals to members’ health plans and encourage loved ones’ participation • Ensure members’ primary care physicians are aware of their health and quality of life

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Multi-state licenses (preferred) • MD or DO qualification • Ability to travel locally and within licensed states, as needed • Ability to conduct virtual visits as needed • Ability to safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
