📋 Description

• Provide in-home health evaluations in members’ homes • Conduct approximately 45-minute visits with one-on-one health conversations • Review medications and medical history • Perform physical evaluations • Perform additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Provide Signify Health services, including Diagnostic & Preventive Services such as spirometry, DEE and bone density screening • Conduct virtual visits as needed • Build trusted relationships and help improve health outcomes • Travel locally and within licensed states as needed • Connect members to appropriate care and make referrals to their health plans • Encourage participation of members’ loved ones and ensure primary care physicians are aware of members’ health and quality of life

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Multi-state licenses (preferred) • Must be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants • Ability to travel locally and within licensed states, as needed • Ability to conduct virtual visits as needed • MD or DO qualification explicitly indicated by the required and preferred qualifications section
