📋 Description

• Provide full-time In-Home Health Evaluations in members’ private residences • Conduct approximately 45-minute visits including medication and medical history review and physical evaluation • Perform additional diagnostic tests when ordered by the health plan, such as diabetic eye exams, spirometry, and bone density screening • Answer health questions and identify connections to appropriate care • Use an iPad and clinical workflow for each evaluation • Travel locally and within licensed states as needed • Conduct virtual visits as needed • Provide Signify Health Diagnostic & Preventive Services products • Build trusted relationships and help improve health outcomes • Coordinate referrals to members’ health plans and encourage involvement of loved ones when appropriate • Ensure primary care physicians are aware of members’ health and quality of life • No requirement to prescribe medicine, order lab tests, or alter current treatment regimens

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Ability to work locally and within licensed states as needed • Ability to conduct virtual visits as needed • Ability to provide Diagnostic & Preventive Services, including spirometry, diabetic eye exams, and bone density screening • Ability to safely perform duties in private residences that may contain household pets, secondhand smoke, allergens, or irritants • MD or DO qualification is specified by the qualifications heading • Multi-state licenses preferred
