📋 Description

• Provide in-home health evaluations in health plan members’ residences • Conduct approximately 45-minute visits, connecting one-on-one and answering health questions • Review medications and medical history • Perform physical evaluations • Provide additional diagnostic and preventive services when ordered, including diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Build trusted relationships and help bridge gaps in care • Travel locally and within licensed states, up to 100% of the time • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services products • Make referrals to health plans and encourage participation of members’ loved ones when appropriate • Help ensure primary care physicians are aware of members’ health and quality of life • Visit people nationwide as part of a network of purpose-driven clinicians

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Able to obtain and maintain multi-state licenses in coverage area, as required • Able to cover all counties within California and flex to other areas of business need • MD or DO qualification • Able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and airborne allergens or irritants • Able to travel locally and within licensed states, up to 100% of the time • Willingness and ability to accommodate overnight hotel stays, car rentals, and/or flights as required • Ability to conduct virtual visits as needed • Anticipated availability of 40 hours per week
