📋 Description

• Provide in-home health evaluations in members’ private residences • Conduct approximately 45-minute visits, connecting one-on-one and answering health questions • Review medications and medical history • Perform physical evaluations • Conduct additional diagnostic tests when ordered by the health plan, including diabetic eye exams, spirometry, and bone density screening • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Signify Health Diagnostic & Preventive Services • Travel locally and within licensed states • Build trusted relationships and help improve health outcomes • Support connections to appropriate care and make referrals to health plans as appropriate

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Able to obtain and maintain multi-state licenses in coverage area, as required • MD or DO qualification • Able to travel locally and within licensed states • Able to safely perform duties in homes that may contain household pets, secondhand smoke, and airborne allergens or irritants • Willingness and ability to travel up to 100% of the time, including possible overnight hotel stays, car rentals, and/or flights
