📋 Description

• Provide in-home health evaluations for health plan members • Engage with people in their homes and help bridge gaps in care • Conduct approximately 45-minute visits • Review medications and medical histories • Perform physical evaluations • Perform additional diagnostic tests such as diabetic eye exams, spirometry, and bone density screening when ordered • Use an iPad and intuitive clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Diagnostic & Preventive Services products • Visit people nationwide and connect them to appropriate care • Build trusted relationships and improve health outcomes • Travel locally and within licensed states

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Able to obtain and maintain multi-state licenses in coverage area, as required • MD or DO qualification • Able to safely and comfortably perform duties in homes that may contain household pets, secondhand smoke, and airborne allergens or irritants • Able to travel locally and within licensed states • Able to conduct virtual visits as needed • Able to use an iPad and clinical workflow • Willingness to travel up to 100% of the time, including possible overnight hotel stays, car rentals, and/or flights
