📋 Description

• Provide in-home health evaluations in members’ residences • Conduct approximately 45-minute visits with one-on-one health discussions • Review medications and medical history • Perform physical evaluations • Provide additional diagnostic tests when ordered by the health plan, including diabetic eye exams, spirometry and bone density screening • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Signify Health Diagnostic & Preventive Services • Travel locally and within licensed states across a defined coverage area • Build trusted relationships and help bridge gaps in care • Make referrals to members’ health plans and encourage participation of loved ones when appropriate • Ensure primary care physicians are aware of members’ health and quality of life

🎯 Requirements

• MD or DO • Active, unrestricted license(s) in coverage area(s) • Able to obtain and maintain multi-state licenses in coverage area, as required • Able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants • Able to travel locally and within licensed states • Able to conduct virtual visits as needed • Able to use an iPad and intuitive clinical workflow • Ability to travel up to 100% of the time, including potential overnight hotel stays, car rentals and/or flights • Anticipated weekly hours: 40
