📋 Description

• Provide in-home health evaluations for health plan members • Travel primarily across a defined coverage area, potentially up to 100% of the time • Conduct approximately 45-minute visits • Engage one-on-one with members and answer health questions • Review medications and medical history • Perform physical evaluations • Provide additional diagnostic tests when ordered, including diabetic eye exams, spirometry and bone density screening • Use an iPad and intuitive clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Diagnostic & Preventive Services products • Build trusted relationships and help bridge gaps in care • Make referrals to members’ health plans and encourage loved-one participation when appropriate • Ensure primary care physicians are aware of members’ health and quality of life

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Able to obtain and maintain multi-state licenses in coverage area, as required • Must be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants • MD or DO qualification is explicitly referenced in the required and preferred qualifications heading • Ability to travel locally and within licensed states, up to 100% of the time • Overnight hotel stays, car rentals and/or flights may be required • Ability to conduct virtual visits as needed • Ability to use an iPad and clinical workflow • Ability to work 40 hours per week
