📋 Description

• Provide In-Home Health Evaluations in members' homes • Travel up to 100% of the time across a defined coverage area • Conduct approximately 45-minute visits • Engage one-on-one with members and answer health questions • Review medications and medical history • Perform physical evaluations • Provide additional diagnostic tests when ordered by the health plan, including diabetic eye exams, spirometry and bone density screening • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Signify Health Diagnostic & Preventive Services • Build trusted relationships and help bridge gaps in care • Make referrals to members' health plans and encourage participation of loved ones • Ensure primary care physicians are aware of members' health and quality of life • Travel locally and within licensed states • Overnight hotel stays, car rentals and/or flights may be required

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Able to obtain and maintain multi-state licenses in coverage area, as required • Must be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants • Ability to travel locally and within licensed states • Ability to conduct virtual visits as needed • MD or DO qualification is required by the stated role title and qualifications
