📋 Description

• Provide in-home health evaluations for Signify Health members • Conduct approximately 45-minute visits, including medication and medical history reviews and physical evaluations • Perform additional diagnostic tests when ordered by the person’s health plan, including diabetic eye exams and spirometry • Use an iPad and clinical workflow during each evaluation • Answer health questions and help bridge gaps in care • Build trusted relationships and help improve health outcomes • Travel locally and within licensed states as needed • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services such as spirometry, diabetic eye exams, and bone density screening • Work in members’ private residences

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Multi-state licenses (preferred) • Able to cover the primary location: Penobscot, ME • Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other airborne allergens or irritants • Nurse Practitioner or Physician Assistant qualification
