📋 Description

• Coordinate care to support optimal member health outcomes • Provide member education and facilitate access to appropriate healthcare services • Use care management tools and data analysis to assess member needs and identify risks • Develop and implement effective care plans • Collaborate with internal teams, providers, and external resources to address barriers • Improve quality of care and ensure services align with member benefits • Promote member engagement, informed decision-making, and healthy behavior changes through communication, coaching, and motivational techniques • Monitor care plan progress and document activities • Ensure compliance with regulatory standards, company policies, and quality management guidelines • Travel throughout the assigned Florida field locations up to 75% of the time

🎯 Requirements

• Bilingual English and Spanish required • Must successfully pass verbal and written language assessments in both languages • Must have reliable transportation and be willing and able to travel up to 75% of the time • Must reside in North Miami, Miramar, Aventura, Pembroke Pines, Hialeah, or Flagler, Florida • At least 1 year of experience in a behavioral health or long-term care setting • Proficiency in Microsoft Office Suite and computer use • Bachelor’s Degree or non-licensed Master’s level clinician required • Degree in behavioral health or human services, such as psychology, social work, or marriage and family counseling • Experience in discharge planning, case management, or managed care preferred • Ability to multitask, prioritize, and adapt in a fast-paced environment preferred • Advanced verbal and written communication skills in English and Spanish preferred
