📋 Description

• Develop proactive plans of care to address identified issues and improve short- and long-term outcomes • Assess member needs and benefits using clinical tools and information/data review • Apply clinical judgment to reduce risk factors and barriers and address complex health and social indicators • Conduct assessments using information from claims and other sources, including co-morbidities and multiple diagnoses affecting functionality • Use a holistic approach to determine referrals to clinical resources and interdisciplinary team members • Collaborate with supervisors, healthcare stakeholders, members, providers, and community organizations to overcome barriers and meet goals • Present cases at interdisciplinary case conferences • Perform case management activities, including assessing, planning, implementing, and coordinating care • Follow regulatory and company case management policies and procedures • Use motivational interviewing to maximize member engagement and assess health status and needs • Meet members face to face in the assigned Monmouth/Ocean County area

🎯 Requirements

• Minimum 3+ years of clinical practice experience • Active and unrestricted RN licensure in the state of New Jersey required • Willing and able to travel 25-50% of the time using your own vehicle • Reliable transportation required • Must reside close to or within Monmouth/Ocean County, New Jersey • Dedicated workspace free of interruptions • Separate dependent care arrangements during work hours • Certified Case Manager preferred • Minimum 2+ years Care Management, Discharge Planning and/or Home Health Care Coordination experience preferred • Confidence working at home/independent thinker • Excellent analytical and problem-solving skills • Effective communication, organizational, and interpersonal skills • Ability to work independently • Effective computer skills, including navigating multiple systems and keyboarding • Proficiency with MS Word, Excel, Outlook, PowerPoint, and proprietary applications • Bilingual preferred • Associate's Degree required • Bachelor's degree preferred
